Combined Therapy for Peyronie¢¥s Disease


Abstract
Peyronie¢¥s disease or plastic induration of the penis is characterized by fibrous infiltration of the intereavernous septum of the dorsum of the penis and usually seen in men of middle age. The common clinical manifestations of the condition are curveture of the penis on erection depending on the extent of the fibrosis, a lump on the penis, painful erecton, incapability of erection distal to the process and interference with coitus. Therefore, these compaints frequently cause impotence and result in an embarrassing physical and psychological disability.
During recent years, various forms of treatment have been advocated for the disease. Steroid therapy, irradiation, vitamin E, potassium aminobenzoate, ultrasonic, iontophoreis, etc. are thought to be effective, and they are usually used both alone and in conjunction with other treatments.
Four patients with Peyronie¢¥s disease were treated by combination methods since middle of last year. The age of the patients varied from 55 to 59, average being 56. Each patient had history of dorsal angulation and pain of the penis on erection due to the cordlike fibrotic plaques. On examination, the indurations were situated in the dorsum of the penis beneath the dorsal vessels in all patients. The plaques could be-palpated near the glans extending to the midshaft of the penis in three cases, and near the glans and at the base of the penis in 1 case. The individual plaques measured 2 to 5 cm in length and a pencil size in thickness. In two out of the 4 cases intercourse was possible but not satiafactory. The concomitant disorders were diabetes in 1 case, bilateral tuberculous epididymitis in I case, verumontanitis and B.P.H. in 1 case. They ha-: e been discovered the plaques 1 to 6 months because of the incidious onset and slow progression of the conditions.
They received various types of conbined treatments for 2 to 9 months. The treatments composed of prednisolone, vitamin E, kimo-tab (antiinflammatory and antifibrotic agent, irradiation and local application of potassium paraaminobenzoate ointment. The plaque and painful erection disappeared in I case, pain on erection decreased but the induration persisted in 1 case, softening of the plaque was resulted in 1 case, and the fibrotic plaque became smaller and softer though it did not disappeared entirely in I case.
